Long-Term Effects of HLA Matching on Graft Survivals in Cyclosporine Treated Living Kidney Transplants

Abstract
HLA matching continues to stand out as an important factor in renal transplantation de-spite improvement in immunosuppresion. Therefore, we have analyzed the data of 508 cyclosporine-treated primary living kidney transplants(LRD=218, NRD=290) performed in Catholic Medical Center from 1984 to 1991 to detemine beneficial effect on long-term graft survival of HLA matching grades.
An impressive 24% difference in kidney graft survival was observed at 5 years between the recipients who recieved 0 and 2 DR mismatches(85% vs. 61%; P=0.0001). The 5-year kidney graft survivals in the 0, 1, 2, 3 B+DR mismatches were 80%, 85%, 76% and 79%, which were significantly different from 56% survival in the 4 B+DR mismatched group(P =0.0249). The 5-year kidney graft survivals in the 0, 1, 2 DR mismatched LRD group were 79%, 76%, 67% respectivcly(P=0.0415), and in the NRD were 94%, 82%, 54% respectively(P= 0.0024). However, the effect of HLA-A, B, A+B, A+DR and A+B+DR mismatching grades showed a little difference between the groups with different mismatches number.
In conclusion, matching for the HLA-DR, B+DR significantly improved kidney graft survivals in the CsA-treated living primary kidney transplant recipients.
